Patient Confidentiality and Access To Medical Records
If you would like to know how your information is used, who may have access to that information or your rights to see and obtain copies of your records, either download the attached documents or call at reception and ask for a copy of: Looking After Your Health Record and/or Access to information.
From April 2015 patients are able to view a summary of their medical record online. The data will include: Medication, Allergies and Adverse reactions.
How do I register to view my record?
Call at reception and bring some photo identification with you (such as a passport, driving licence or student card) as we will need to verify your identification. We will not be able to give you your access letter without this ID verification.
If I have already completed the application form to access other online services such as making or cancelling an appointment, ordering a repeat prescription, will I automatically be able to view my medical record summary?
No. To ensure your data is kept secure, you will need to complete the form again and provide photo ID.
For security reasons we cannot give out account details by phone, email or post.
Once the terms and conditions of online access have been agreed, you will be provided with login details. It is your reposnsibilty to keep your login details and password safe and secure.

This includes, in particular, any suspension of the service resulting from maintenance and upgrades to our systems or those of any party used to provide the service. You must keep your personal details secret and take all reasonable precautions to prevent the fraudulent use of your personal details. We reserve the right to change the service from time to time and shall give you notice of any material changes. We may, where we consider it appropriate for you or your protection, suspend, withdraw or restrict use of the service or any part of the service. We will tell you as soon as practicable if we take such action. We reserve the right to vary these terms and conditions.
