Non-urgent advice: Triage form for GP appointment requests and admin queries

Please select the following link if you require a GP appointment or have an administration query


All patients calling to request a GP appointment will be asked to complete the quick and easy online form.

Patients who do not have access to the internet or are unable to use the form can still call the surgery and one of our reception team will complete the form with you, this will then be sent to the doctor in the same way as patients using the online link.

Once the form is submitted, a doctor will review the form and determine the most appropriate action based on the information provided in the form. You will receive a response within one working day.

Please note that the form is only accessible between 07:30 am and 17:30 pm Monday to Friday.
